JOB SUMMARY
This position participates in the development and implementation of appropriate and effective Individual Support Plans (ISP) for persons served by the agency.
MAJOR DUTIES
• Participates as a member of the individual support team; works with persons, guardians, and caregivers to develop and implement appropriate and effective ISPs that accomplish outcomes desired; participates in initial and annual meetings to review and assess appropriateness and effectiveness of the ISP.
• Ensures the completion of all require reports and assessments.
• Identify individual’s needs utilizing input from the individual, guardians, care givers, staff, and written assessments by other professionals.
• Assists in the development of training goals, service objectives, and behavior goals and objectives; prioritizes desired outcomes.
• Plans, develops, and implements curriculum and instruction of programs, services, and activities to reflect the goals and objectives of the person as identified in ISPs; identifies how training, services, and activities will be provided.
• Reviews ISPs, behavior plans, and training data monthly to evaluate each person’s response to training/services/activities and recommend modifications as necessary.
• Serves as an advocate for persons served.
• Follows-up on emergency concerns.
• Completes and maintains written reports in an accurate and timely manner and in accordance with policies and standards; documents conversations with parents, guardians, care givers, agency staff, and others and provides information to Case Manager; completes and submits to Case Managers all required reporting information.
• Ensures copies of ISPs are distributed to instructional staff; ensures instructional staff training data reflect the goals and objectives identified on ISPs.
• Participates in the development and implementation of goals, objectives, and plans; provides ISP orientation to instructional staff; provides instructional staff with required information for admission,
transfer, or discharge; submits required information to Case Manager’s for completion of quarterly narrative, ISP quarterly updates, and other functions.
• Performs Instructor duties as needed.
• Administers medications as required.
• Completes required in-service training; attends and participates in staff meetings, workshops, and conferences as directed.
• Maintains certification in CPR, First Aid, Medication Assistance (MAC), and CPI.
• Maintains and develops schedules and calendars.
• Accompanies individuals on field trips.
• Performs other duties and responsibilities deemed necessary; not otherwise specified.
KNOWLEDGE REQUIRED BY THE POSITION
• Knowledge of agency policies, Alabama Department of Mental Health guidelines and Medicaid guidelines.
• Knowledge of individual assessment principles and practices.
• Knowledge of ISP development and implementation principles.
• Knowledge of computers and job-related software programs.
• Skill in the development of instructional plans.
• Skill in problem solving.
• Skill in prioritizing and planning.
• Skill in interpersonal relations.
• Skill in oral and written communication.
SUPERVISORY CONTROLS
The Director of Day Programs supervises and assigns work in terms of general instructions. The supervisor spot-checks completed work for compliance with procedures, accuracy, and the nature and propriety of the final results.
GUIDELINES
Guidelines include agency policies, Alabama Department of Mental Health guidelines, and Medicaid guidelines. These guidelines require judgment, selection, and interpretation in application.
COMPLEXITY/SCOPE OF WORK
• The work consists of varied professional assessment and plan development duties. Strict regulations combined with the unique needs of individuals served contribute to the complexity of the position.
• The purpose of this position is to develop and implement appropriate and effective ISPs for persons served. Successful performance contributes to the delivery of effective services to individuals.
CONTACTS
• Contacts are typically with co-workers, other agency personnel, guardians, care givers, staff, individuals served, and members of the general public.
• Contacts are typically to exchange information, motivate persons, resolve problems, and provide services.
PHYSICAL DEMANDS/ WORK ENVIRONMENT
• The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable an employee to perform essential job functions.
• The work is typically performed while sitting at a desk or table or while intermittently sitting, standing, or stooping. The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds, and occasionally lift and/or move over 25 pounds. Specific vision abilities required for this job include close vision, distant vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
• The work is typically performed in an office. The employee may be exposed to noise. Work may require the use of protective devices such as masks, goggles, gloves, etc.
SUPERVISORY AND MANAGEMENT RESPONSIBILITY
This position has direct supervision over assigned Instructors.
MINIMUM QUALIFICATIONS
• Knowledge and level of competency commonly associated with the completion of a baccalaureate degree in a course of study related to the occupational field.
• Experience sufficient to thoroughly understand the work of subordinate positions to be able to answer questions and resolve problems, usually associated with one to three years’ experience or service.
• Meets Alabama Department of Mental Health QDDP qualifications.
• Possession of a valid driver’s license, good driving record and required to obtain a CDL.
