Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

An excellent direct hire opportunity is available for an experienced Home Infusion Pharmacist to join a leading infusion pharmacy team in the Birmingham area . This role is ideal for a pharmacist passionate about providing high-quality patient care in a collaborative, patient-focused environment.

Schedule: Monday Friday, full-time (8:30am5:00pm or 9:00am5:30pm)
Flexibility to stay later as needed

Overview:
The pharmacist will deliver safe and appropriate pharmacy services to patients in the alternate care/home infusion setting. This includes assessing therapy appropriateness, dispensing medications and supplies, educating patients and caregivers, and monitoring ongoing therapyall while ensuring compliance with state, federal, and accreditation standards.

Key Responsibilities:

• Provide professional and ethical pharmacy practice in accordance with all regulatory and company standards.

• Evaluate patient eligibility and therapy appropriateness for home infusion services.

• Design and manage individualized patient care plans with defined therapy goals.

• Dispense medications, IV supplies, and equipment accurately and efficiently.

• Monitor lab results, perform pharmacokinetic dosing, and document therapy outcomes.

• Educate patients and caregivers on proper medication use and infusion procedures.

• Ensure USP <797> compliance for all sterile compounding and cleanroom operations.

• Supervise and delegate work to pharmacy technicians, interns, and students.

• Participate in patient care meetings and provide ongoing drug information support.

• Participate in on-call rotation and respond to patient needs as required.

Qualifications:

• Bachelor of Science in Pharmacy (PharmD or Masters preferred).

• Active and clear Alabama Pharmacist license required.

• Minimum 1 year of recent home infusion experience .

• Strong knowledge of IV admixture services, sterile compounding, and infusion therapy.

• Excellent communication, clinical judgment, and organizational skills.
