Discharge Planner – Inpatient Behavioral Health

The Sanctuary is seeking a full-time Discharge Planner to join our inpatient behavioral health team serving adult and older adult patients.

The Discharge Planner plays an important role in helping patients transition safely from hospitalization to the next level of care. This position works closely with patients, families, social services, nursing, providers, utilization management, treatment team members, and community partners to coordinate discharge needs and reduce barriers to successful transitions.

Compensation:

1. Salary: $20-$24/hr based on experience

Key responsibilities include:

• Coordinating discharge planning throughout hospitalization
• Scheduling follow-up appointments and aftercare services
• Completing and tracking referrals
• Assisting with nursing facility, residential, and community placements
• Coordinating transportation and community resources
• Participating in interdisciplinary treatment team meetings
• Identifying and escalating barriers that may delay discharge
• Maintaining timely and accurate discharge-planning documentation

Qualifications:

• Associate degree in Human Services, Social Work, Psychology, Behavioral Health, Healthcare, Nursing, Sociology, Healthcare Administration, Criminal Justice, or a related field required
• Bachelor's degree preferred
• Behavioral health, healthcare, case management, discharge planning, social services, or care coordination experience preferred
• Inpatient psychiatric or acute-care experience strongly preferred
