An opportunity is available for an experienced Infusion Pharmacist to provide exceptional pharmacy services in a home infusion care setting. This direct hire position requires a professional dedicated to delivering safe, effective, and patient-centered pharmaceutical care. The role focuses heavily on intravenous therapy and working closely with healthcare teams to ensure optimal patient outes.

Key Responsibilities:

• Assess patient suitability for home infusion therapy and develop individualized pharmacy care plans.
• Dispense medications, intravenous admixtures, and related supplies accurately and in pliance with regulatory standards.
• Monitor patient progress, document therapy outes, and adjust treatment plans as necessary.
• Perform pharmacokinetic dosing and evaluate laboratory results to inform dosing decisions.
• Educate patients and caregivers on medication administration, infusion techniques, and safety measures.
• Collaborate with physicians, nurses, and healthcare professionals to support prehensive care.
• Oversee pharmacy support personnel, delegate tasks, and assist in training.
• Ensure pliance with USP standards and all state and federal pharmacy regulations.
• Participate in patient care conferences and be involved in the on-call rotation when required.

Qualifications and Desired Experience:

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) or Masters degree preferred.
• Current pharmacist license in the state of employment.
• Minimum of two years recent acute-care experience or at least one year in home infusion therapy.
• Strong knowledge and experience in intravenous admixture services and intravenous therapy.
• Excellent munication, organizational, and problem-solving skills.
• Ability to effectively supervise pharmacy staff and work collaboratively with clinical teams.
• Flexibility with work schedules is essential.
