Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A direct hire opportunity is available for an experienced infusion pharmacist specializing in home infusion pharmacy services. This role involves prehensive management and delivery of safe, effective pharmacy care to patients receiving treatment in alternate care settings.

Location: Prattville, AL
Schedule: Monday Friday, 8:00 AM 5:00 PM
Job Type: Direct Hire

Responsibilities:

• Ensure ethical pharmacy practice in pliance with all regulatory, policy, and professional standards.
• Assess patient suitability for home infusion therapy and appropriateness of prescribed treatments.
• Design individualized pharmacy care plans, including dosage remendations and monitoring of therapy progress.
• Dispense medications, supplies, and manage equipment such as infusion pumps, ensuring correct preparation and administration.
• Maintain accurate documentation of patient care, including munication with prescribers and caregivers.
• Oversee pharmacy pounding activities, adhering to USP standards and safety regulations.
• Participate in interdisciplinary patient care conferences and support pharmacy support personnel.
• Provide pharmaceutical education and support to patients, caregivers, and healthcare providers.
• Participate in the on-call rotation and manage tasks related to urgent patient needs as required.

Qualifications:

• Bachelor of Science in Pharmacy required; PharmD or Masters degree preferred.
• Current pharmacist license in the state of employment.
• Minimum of 2 years acute care pharmacy experience or 1 year in home infusion therapy.
• IV admixture and infusion therapy expertise essential.
• Strong problem-solving skills and supervisory experience.
• Effective munication and organizational skills.
