Program: Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID)
Employment Type: Full-Time /Non-Exempt
Reports To: ICF Enrichment Coordinator Position SummaryThe Life Skills Coach / Activities & Vocational Support staff member provides direct support to adults with intellectual and/or developmental disabilities by assisting with daily living skills, implementing active treatment plans, facilitating meaningful recreational activities, and supporting pre-vocational and vocational skill development. This position promotes independence, engagement, dignity, and quality of life in accordance with each resident’s Individual Program Plan (IPP), behavior support plans, and applicable ICF/IID regulations.Direct Support & Life Skills DevelopmentAssist residents with activities of daily living (ADLs) as needed
Support independent living, social, communication, and self-advocacy skills
Know each resident personally to support individual needs and preferences
Fill in as a resident aide as needed to ensure continuity of careActivities & Active TreatmentImplement activity portions of active treatment plans for ICF/IID residents
Plan and deliver fun, safe, and engaging daily activities, including weekends
Assist residents with activities outlined in the IPP and Activities Binder
Develop and display thematic and holiday decorations throughout the facilityPre-Vocational & Vocational Skills DevelopmentPlan and implement pre-vocational and vocational training activities
Teach job-related skills including task completion, time management, communication, and teamwork
Assess resident interests, strengths, and support needs related to vocational participation
Incorporate vocational goals into residents’ IPPs in collaboration with the interdisciplinary team
Coordinate on-site vocational activities and community-based vocational or volunteer opportunities
Document participation, skill acquisition, and progress toward vocational goalsCommunity Integration & TransportationTransport residents to on-campus and off-campus activities using facility vehicles
Transport residents to medical, vocational, and personal appointments
Shop with and for residents in the local community to promote independenceDocumentation & CompliancePrepare documentation related to IPPs, behavior support plans, and vocational goals
Complete reports required by licensing and regulatory entities
Accurately document daily activities and resident progressCommunication & Team CollaborationReport complaints, incidents, or concerns to the Program Director or Program Coordinator
Participate in ICF/IID and Rainbow Omega meetings or committees as requested
Collaborate with interdisciplinary team members and external partnersAdministrative & Other DutiesOperate office equipment including computer, printer, and copier
Submit supply requests to the Program Coordinator
Perform other duties as assignedQualifications Required:
High school diploma or GED
Ability to work effectively with adults with intellectual and/or developmental disabilities
Valid driver’s license with acceptable driving record
Preferred:
Experience working in an ICF/IID or vocational setting
Coursework in human services, vocational rehabilitation, or related field
CPR, First Aid, CPI certification or willingness to obtainPhysical & Work RequirementsAbility to assist with lifting, transferring, and mobility support
Ability to stand, walk, bend, and engage in physical activities throughout the shift
Willingness to work evenings, weekends, holidays, and flexible schedules
