Job Summary

The Discharge Planner (RN, LPN, or Social Worker) coordinates safe and timely patient discharges across the Hoover and greater Birmingham, Alabama area , ensuring smooth transitions of care and access to appropriate post-acute services.

Responsibilities

1. Coordinate discharge planning and care transitions
2. Collaborate with nursing, therapy, physicians, and families
3. Arrange home health, DME, rehab, and placement services
4. Connect patients with Birmingham-area community resources
5. Manage documentation and ensure timely, accurate records

Qualifications

1. Active Alabama license (one of the following required):
2. Registered Nurse (RN)
3. Licensed Practical Nurse (LPN)
4. LMSW or LCSW (Social Worker)
5. Experience in discharge planning, case management, or care coordination preferred
6. Strong communication, organization, and problem-solving skills
