Forest Manor Health and Rehab’s 40-bed, post-acute rehabilitation facility is designed as a luxury addition to the existing Forest Manor Nursing Home, located in Northport, Alabama. Our progressive design constitutes a bold, but necessary, departure from the semiprivate institutional feeling that characterized its predecessor. Recognizing the need for independence, and the high expectations of the aging Baby Boomer generation, our facility was developed to re-create the atmosphere is a resort facility. We place a high importance on attracting dynamic healthcare professionals who share a philosophy of quality patient/clinical care in a welcoming, respectful, and cohesive team environment.

Responsibilities:

· Evaluate, plan and administer treatment to residents and patients with temporary or long term disabilities to relieve pain, restore or improve function, and promote healing

· Assure all treatment is delivered in accordance with an established plan of care

· Provide clinical support and instruct patients, families, and caregivers

· Monitor patient response to treatment intervention

· Complete required forms and documentation in accordance with company policy and state/federal regulations
