Nurse Manager - Remote
Conduct outbound calls to new and existing patients, providing proactive care and support.
Engage in text-based conversations to check in, answer questions, and build patient relationships.
Monitor patient symptoms and behaviors, identifying any changes that require further attention.
Collaborate with physicians and specialists to ensure coordinated care.
Educate patients on health maintenance, disease prevention, and medication adherence.
Facilitate referrals to appropriate healthcare providers or medical facilities when needed.
Maintain accurate and up-to-date patient records.
Offer guidance, encouragement, and emotional support to patients and their families.
What We're Looking For:
Active RN or LPN license in [state(s), if applicable].
Experience in nursing, chronic disease management, or telehealth is preferred.
Comfortable using medical software and technology for remote patient interactions.
Ability to build rapport with patients through phone and text communication.
Strong critical-thinking and problem-solving skills.
Ability to thrive in a fast-paced, remote work environment with minimal supervision.
