**Job Description**
We are looking for a **Program Specialist** to coordinate care and bridge gaps between programs, schools, hospitals, providers, families, and communities for children with complex needs. This position will be **full-time contract** and **remote** .
**What You'll Do**
+ Review referred cases to determine acceptance and provide feedback for denied cases
+ Leverage system-wide expertise to identify needs and service options for the child and family
+ Consult and collaborate with relevant parties (e.g. teachers and case managers) to gain an understanding of the child's and family's needs
+ Connect with families as necessary to understand their needs and ensure alignment and support throughout the process
+ For NDI Step-Down, support cases from admission to NDI through treatment, ensuring a smooth transition post-discharge
+ Review treatment plans and collaborate with any existing case managers, providers, and state agencies' personnel to coordinate step-down/discharge plans and proactively arrange next steps
+ Provide program referrals including management of required documentation and attendance in relevant referral meetings
+ Remain engaged in the case on an ad-hoc basis for at least six months post-discharge to monitor progress (for NDI Step-Down) or post-plan development and implementation (for School-Based Prevention)
+ Maintain detailed case notes, track data for state reporting, and identify barriers within systems that limit access to services, providing this feedback to appropriate agencies and partners
**What You'll Need**
Required:
+ Meet one of the following degree and experience combination requirements
+ Master's degree in social work, education, or other relevant field AND at least 3 years of relevant work experience
+ Bachelor's degree in social work, education, or other relevant field AND at least 5 years of relevant work experience
+ Advanced knowledge of Indiana programs that serve children across physical health, child services, education, disabilities, mental/behavioral health systems, etc. with the ability to understand the programs and navigate options to improve outcomes for children and families
+ Experience working with children with high acuity needs, such as complex emotional, behavioral, and mental health conditions, and supporting their families and other professionals in their lives
+ Ability to formulate and monitor treatment plans and coordinate services for challenging and complex cases often involving multiple diagnoses and multi-system interaction
+ Critical thinker and strong judgement skills with the ability to analyze complex situations and develop innovative, "outside-the-box" solutions to overcome challenges
+ Skilled at navigating interagency dynamics, adapting to evolving needs, and ensuring a child-and-family centered approach
+ Excellent communication skills enabling effective collaboration with families, service providers, agency personnel, and other stakeholders
+ Proficiency in preparing accurate reports and tracking data related to the pilot program
+ Sensitivity and ability to work with families from multiple cultural and ethnic backgrounds, respecting differences in child-rearing practices, relationships, and needs
+ Adherence to confidentiality standards, with the ability to handle personal health information in compliance with HIPAA, FERPA and state policies, and knowledge of state and/or federal laws, regulations, guidelines, and standards affecting special education, child services, Medicaid programs and other social service programs, and juvenile justice
Preferred:
+ At least 5 years of experience working with children with high acuity needs and their families
+ Past experience working with the State of Indiana and the involved agencies listed above (FSSA, DCS, DOE, NDI, LEAs)
+ Lived experience in areas related to the needs and services of children with high acuity needs
**Physical Demands**
+ Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state and local standards
+ Sedentary work that involves sitting or remaining stationary most of the time with occasional need to move around the office to attend meetings, etc.
+ Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor
The pay range for this position is $55.00 per hour (USD). Exact compensation may vary based on several factors, including location, experience, and education. Benefit packages for this role includes medical, dental, and vision insurance, as well as 401k retirement account access. Employees in this role may also be entitled to paid sick leave as provided by applicable law.
