Position Overview:
As a Home Infusion Pharmacist, you will provide safe and effective pharmacy services to patients in alternate care and home infusion settings. You will assess therapy appropriateness, dispense medications and supplies, educate patients and caregivers, monitor therapy progress, and ensure compliance with all regulatory standards, including USP <797> .

Key Responsibilities:

• Deliver ethical, compliant pharmacy practice in line with regulations and professional standards

• Evaluate patients for home care suitability and therapy appropriateness

• Design and manage patient-specific care plans and therapy goals

• Dispense medications, supplies, and medical equipment for home infusion

• Monitor lab results, perform pharmacokinetic dosing, and document therapy outcomes

• Educate patients and caregivers on safe medication use and infusion techniques

• Ensure USP <797> compliance for compounding and clean room activities

• Supervise and delegate tasks to pharmacy support staff, interns, and students

• Participate in patient care conferences and provide drug information

• Respond to patient needs as part of on-call responsibilities

Qualifications:

• Bachelor of Science in Pharmacy (PharmD or Masters preferred)

• Active Alabama Pharmacist License with no negative history

• Minimum 1 year recent home infusion experience

• Strong knowledge of IV admixture services and infusion therapy

• Excellent communication, problem-solving, and organizational skills

Schedule:
Full-time, MondayFriday, typically 8:30am 5:00pm or 9:00am 5:30pm, with occasional flexibility required
