As Close As Family is a locally managed, Alabama‑based in‑home care provider serving seniors, families, and veterans. We partner with VA and other payer programs to help veterans and surviving spouses access benefits and in‑home care services .

Position overview

We are hiring a PRN Registered Nurse to complete Veteran admission assessments, create individualized plans of care, provide oversight of caregivers, and conduct in‑home reassessments every 60–90 days. This is an as‑needed PRN role compensated on a per‑event/visit (per event) basis. The RN in this position will serve clients in Cherokee County.

Key responsibilities

• Perform comprehensive Veteran admission assessments in the home (medical history, functional status, meds, safety risks, social supports, veteran/family goals).
• Develop individualized, person‑centered plans of care that reflect the veteran’s needs and family requests.
• Document assessment findings and care plans thoroughly and submit any VA support documentation as required.
• Provide clinical oversight and direction to caregivers, including in‑home observation and coaching.
• Conduct in‑home reassessments every 60–90 days (or sooner if condition changes) to evaluate ongoing appropriateness of the care plan and identify additional needs.
• Communicate with family members, primary care providers, VA contacts, and internal staff about care status and plan changes.
• Maintain timely, accurate documentation of visits, visits completed, and communications per company policy.
• Follow company compliance, safety, and clinical standards.

Schedule & compensation

• PRN / as‑needed schedule — flexible shifts based on client needs and RN availability.
• Compensation: paid per event (per completed visit). Specific per‑visit rates vary by visit type and will be discussed during hiring.
• Travel required within Cherokee County; mileage reimbursement applies per company policy.

Minimum qualifications

• Current, active RN license in Alabama.
• Minimum 2 years nursing experience preferred (home health, hospice, geriatrics, case management, or related).
• Experience with veteran populations and VA Aid & Attendance helpful but not required.
• Strong assessment, care‑planning, documentation, and communication skills.
• Valid driver’s license, reliable transportation, and proof of required insurance.
• Ability to pass background checks and required screenings.
• Comfortable working independently in clients’ homes and collaborating with families and care teams.

Preferred qualifications

• Previous home‑care case management or supervisory experience.
