We're seeking a skilled and empathetic On-Call Hospice Registered Nurse to join our team, providing critical support and symptom management during after-hours, weekends, and holidays.

Position Overview:

As an On-Call Hospice Registered Nurse, you'll play a vital role in ensuring the comfort and well-being of our patients. You'll be responsible for delivering expert pain and symptom management, adhering to individualized care plans developed by our interdisciplinary team, the patient's attending physician, and our Hospice Medical Director. This is a crucial role, providing essential support and hands-on care when our patients need it most.

Schedule:

Enjoy a balanced 7-on/7-off schedule, allowing for dedicated time off.
Weekday shifts: 5:00 PM to 8:00 AM.
Weekend shifts: Around-the-clock coverage.
Please note: This is a field-based position requiring in-person visits to patients' homes and facilities. This is not a remote position.
Key Responsibilities:

Provide Expert Clinical Care: Deliver timely and comprehensive hospice services, focusing on pain and symptom management, in accordance with the patient's individualized plan of care.
Conduct Thorough Assessments: Perform detailed assessments of patients' physical, emotional, and spiritual needs during scheduled and after-hours home visits. Accurately document assessments, identified problems, nursing interventions, goals, and outcomes.
Coordinate Comprehensive Care: Collaborate with the interdisciplinary team, attending physicians, and other healthcare providers to ensure seamless and coordinated patient care. Maintain detailed documentation of all coordination efforts.
Develop and Implement Care Plans: Actively participate in the development, review, and revision of patient care plans, contributing valuable insights and ensuring effective interventions.
Ensure Continuity of Care: Facilitate smooth transitions between care settings, including home, long-term care facilities, and inpatient facilities, maintaining clear communication with all involved parties.
Communicate Effectively: Maintain open and transparent communication with patients, families, caregivers, and the interdisciplinary team, sharing vital information about patient needs, assessments, interventions, and outcomes.
Educate and Support: Provide ongoing education and support to patients and their families/caregivers, empowering them to understand and navigate the hospice care process.
Supervise and Mentor: Oversee the delivery of patient care provided by Hospice Aides and Licensed Practical Nurses, ensuring adherence to care plans and completing necessary supervisory documentation.
Facilitate Facility Care: Coordinate hospice services for patients residing in long-term care facilities, collaborating with facility staff and participating in care plan meetings as needed.
On-Call Availability: Participate in the on-call rotation, providing responsive and compassionate care during after-hours, weekends, and holidays.
Maintain Hospice Eligibility: Continuously monitor patient eligibility for hospice services and complete all required documentation for certification and recertification.

Qualifications:

Current and unrestricted Registered Nurse (RN) license in the state of application.
Preferred Qualifications:

Experience with Medicare Skilled Nursing.
Familiarity with OASIS documentation.
Minimum of one year of RN experience in a hospice setting.
