Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A direct hire opportunity is available for a skilled pharmacist to specialize in home infusion pharmacy services. This role focuses on delivering safe, compliant, and patient-centered care within an alternate care/home infusion setting.

Key Responsibilities:

• Provide ethical pharmacy services that align with regulatory and professional standards.
• Assess patients eligibility for home infusion therapy and determine the appropriateness of treatments.
• Develop and manage customized care plans aimed at achieving therapy goals.
• Dispense medications, supplies, and necessary medical equipment for home infusion.
• Monitor patients lab results, conduct pharmacokinetic dosing, and document therapy progress.
• Educate patients and caregivers on medication safety and proper infusion techniques.
• Ensure compliance with USP guidelines related to sterile compounding and clean room procedures.
• Supervise pharmacy support personnel, interns, and students.
• Engage in patient care conferences and offer drug information support.
• Participate in on-call rotations and respond to patient needs outside regular hours.

Qualifications:

• Bachelor of Science in Pharmacy required; PharmD or Masters degree preferred.
• Active pharmacist license in Alabama with a clean professional history.
• Minimum one year recent experience in home infusion pharmacy.
• Strong expertise in IV admixture and infusion therapy.
• Excellent communication, problem-solving, and organizational abilities.

Schedule:

• Full-time, Monday through Friday.
• Typical hours 8:30 AM to 5:00 PM or 9:00 AM to 5:30 PM, with flexibility for extended hours as needed.
