In the role, you’ll deliver high-quality, resident-centered care in long-term care settings. Your responsibilities will include administering medications, monitoring vital signs, assessing patient conditions, and supporting activities of daily living. You’ll collaborate with charge nurses, attending physicians, physical and occupational therapists, social workers, and dietary and rehab teams to implement and adjust individualized care plans. You’ll document assessments, progress notes, and interventions in electronic medical records, ensuring accuracy, compliance, and continuity of care. You’ll assist with wound care, pain management, fall prevention, and infection control, while promoting safety, comfort, and respect for every resident’s preferences.
