MDS Nurse ( RN )
Monday–Friday Day Shift | Minimal Direct Care | Clinical Analytics Focus

If that sounds like you, a career as an MDS Coordinator / Resident Assessment Coordinator (RAC) may be the right next step.
This is a Monday-through-Friday nursing position with a strong focus on resident assessment, documentation, clinical analysis, and care planning. Outside of completing resident assessments, the role is primarily administrative and does not require routine bedside care.
What You’ll Do
Under the direction of the Director of Nursing, the MDS Coordinator leads the interdisciplinary resident assessment process and helps ensure each resident receives an accurate, individualized plan of care.
You will use your clinical experience, analytical skills, and attention to detail to:
- Coordinate comprehensive assessments for assigned residents.
- Review clinical documentation and identify changes in resident conditions, needs, and functional abilities.
- Analyze resident information to ensure assessments accurately reflect the care being provided.
- Lead the development and completion of individualized care plans in accordance with company, state, and federal guidelines.
- Collaborate with nursing, therapy, dietary, social services, and other members of the interdisciplinary care team.
- Monitor care plans and recommend updates when a resident’s condition or needs change.
- Identify documentation gaps, inconsistencies, or trends that require follow-up.
- Serve as an internal resource and educator for team members involved in resident assessment and documentation.

Qualifications
- Current Registered Nurse (RN) license in good standing.
- Strong clinical assessment and documentation skills.
- Excellent analytical, organizational, and deductive reasoning abilities.
- Working knowledge of nursing standards of care and medical practices.
- Comfortable using electronic medical records and documentation systems.
- Ability to communicate effectively and collaborate with an interdisciplinary team.
- Previous MDS, long-term care, or post-acute care experience is helpful but not required for the right candidate.
