The Staff Pad has partnered with Russellville Hospital to help hire a dedicated and compassionate Registered Nurse/Field Staff Coordinator for their Home Health department. About the Role:

As a Registered Nurse/Field Staff Coordinator, you'll play a pivotal role in providing and coordinating patient-centered care for individuals in the home health setting. You'll collaborate with physicians, families, and other healthcare providers to ensure personalized care plans are implemented and continually evaluated. This position also offers the opportunity to support intake and quality improvement processes, adding depth to your clinical leadership experience.

Responsibilities Include:

• Performing initial and ongoing patient assessments
  
• Executing physician orders and implementing nursing interventions
  
• Developing and updating individualized plans of care
  
• Supervising home health aides and participating in performance improvement initiatives
  
• Coordinating interdisciplinary care and managing referrals
  
• Assisting in intake coordination and staff scheduling when needed
  

Qualifications:

• Current RN license from the Alabama Board of Nursing
  
• Minimum of 3 months of nursing experience (home health experience a plus)
  
• Strong clinical judgment and patient advocacy skills
  
• Familiarity with Medicare Coverage Guidelines , OASIS , and Home Health Conditions of Participation
  
• Excellent communication and organizational abilities
  

Patient Populations Served:

• Adolescents (13–18)
  
• Adults (19–65)
  
• Geriatrics (65+)
  

Physical & Work Conditions:

• Moderate physical activity including walking, standing, kneeling, and lifting
  
• Exposure to home care environments and occasional blood-borne pathogens
  
• Must be comfortable navigating various home conditions and travel within the service area
