Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

We are seeking an experienced Home Infusion Pharmacist to join a dedicated team providing patient-centered care in the alternate care/home infusion setting. This is a full-time, Monday-to-Friday position with typical hours from 8:30 AM 5:00 PM or 9:00 AM 5:30 PM, with flexibility to extend when needed.

Position Overview:

• Ensure safe and appropriate pharmacy services for patients receiving home infusion therapy.

• Assess therapy appropriateness and design patient-specific care plans.

• Dispense medications, supplies, and equipment while ensuring compliance with regulatory and accrediting standards.

• Educate patients and caregivers on proper medication use and infusion techniques.

• Monitor therapy progress, review lab results, and perform pharmacokinetic dosing as needed.

• Maintain USP <797> compliance for compounding and clean room activities.

• Supervise pharmacy support staff, interns, and students.

• Participate in patient care conferences and provide drug information.

• Participate in on-call rotation to support patient needs after hours.

Requirements:

• PharmD or Bachelor of Science in Pharmacy (Masters preferred).

• Active Alabama Pharmacist license with no negative history.

• Minimum of 1 year recent home infusion experience.

• Strong knowledge of IV admixture services and infusion therapy.

• Excellent communication, organizational, and problem-solving skills.
