Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A direct hire opportunity is available for an experienced Infusion Pharmacist with expertise in home infusion and intravenous admixture services. This role is based in Wetumpka, AL, and requires a candidate who is IV certified and knowledgeable in pharmacy operations within home infusion settings.

Key Responsibilities:

• Provide safe and pliant pharmacy services in an alternate care environment, following pany policies and regulatory standards.
• Assess patient suitability for home infusion therapy and develop individualized pharmacy care plans.
• Review and remend appropriate medication orders, including pharmacokinetic dosing adjustments.
• Dispense medications, supplies, and medical equipment accurately while ensuring authorization and cost-effective delivery.
• Document all patient interactions, therapy progress, and munications with healthcare professionals.
• Ensure adherence to USP standards and proper pounding practices.
• Supervise and mentor pharmacy support staff and participate in patient care meetings.
• Manage daily pharmacy operations, including inventory, billing documentation, and scheduling.

Desired Qualifications:

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy or Masters degree preferred.
• Current pharmacist license in the state of employment.
• Minimum two years of recent acute care experience or at least one year in home infusion therapy.
• Strong knowledge of intravenous admixture and infusion therapy.
• Ability to collaborate effectively with multidisciplinary healthcare teams.
• Excellent verbal and written munication skills.
• Proficient in pharmacy calculations and moderate puter skills.
• Flexible and dependable with strong organizational abilities.
