Desired Qualifications

• Licensed Social Worker in Alabama (Preferred)
• Degree or education in Social Sciences
• Long-term care experience preferred
• Strong communication, documentation, organization, and follow-up skills
• Compassionate, dependable, flexible, and resident-focused

Essential Responsibilities

• Serve as the advocate and liaison for residents’ social and emotional well-being.
• Plan, organize, implement, and evaluate the Social Services Department.
• Assist residents and families with admissions, discharge planning, care transitions, and available community resources.
• Ensure residents’ medically related emotional and social service needs are identified and addressed on an individual basis.
• Maintain compliance with all federal, state, and local regulations governing long-term care facilities.
• Collaborate with the interdisciplinary care team to promote the highest quality of resident care and quality of life.
• Complete timely and accurate documentation while maintaining resident confidentiality.
