Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A direct hire opportunity is available for an experienced infusion pharmacist specializing in home infusion therapy and intravenous admixture services. This role is based in Millbrook, AL, and offers a Monday through Friday schedule from 8:00 AM to 5:00 PM, providing a balanced work-life routine within a collaborative care environment.

Key Responsibilities:

• Deliver safe, effective pharmacy services in the alternate care setting following professional standards and regulatory guidelines.
• Assess patient suitability for home care and the appropriateness of prescribed intravenous therapies.
• Develop individualized pharmacy care plans, including medication order remendations and documentation of patient progress.
• Dispense and manage medications, infusion supplies, and equipment accurately, ensuring patibility and stability.
• Provide patient and caregiver education on medication use, administration techniques, and therapy goals.
• Ensure strict pliance with pounding standards and maintain quality assurance documentation.
• Collaborate with healthcare professionals, participate in patient care rounds, and oversee pharmacy support personnel.
• Participate in on-call duties and contribute to staff training and supervision.

Qualifications and Desired Experience:

• Bachelor of Science in Pharmacy; Doctor of Pharmacy or Masters degree preferred.
• Active pharmacist license in the state of employment.
• At least two years of recent acute-care pharmacy experience or a minimum of one year in home infusion therapy.
• Strong knowledge and hands-on experience with intravenous admixture and therapy services.
• Excellent verbal and written munication skills alongside effective problem-solving and organizational abilities.
• Flexibility in work schedule and ability to supervise technical staff.
