POSITION SUMMARY:
Manage and facilitate the admission and discharges of residents in our Skilled Nursing Facility. Serves as a Social Liaison between residents, families and staff. Communication with the management staff daily is essential. Model customer service principles that promote an appreciation of our residents, families, referral sources and employees.
ESSENTIAL DUTIES AND RESPONSIBILITIES

- Coordinates with referral sources (i.e. hospitals, case managers) the timely clinical and financial review of referred patients.
- Maintains constant communication on status of all referrals with referral sources, families and Health Care Staff.
- Verify Primary and co-insurance prior to admission and negotiate out of network rates on managed care contract referrals
- Assists the family with completion of new admission paperwork and Medicaid applications.
- Prepare daily census
- Ensure timely documentation of all admission records.
- Provide facility tours upon request.
- Inform department managers of new admissions to ensure a safe admission with family and resident satisfaction.
- Complete and maintain Social service charting and documentation to meet resident needs and continue compliance with State and Federal regulations.
- Initiate and maintain current discharge planning for each resident.
- Implement and monitor an effective Behavior Management Program.
- Provide in-service and on-going training for the staff on Behavior Management, resident rights and other social issues.
- Attend all care plan meetings.
- Assure all residents have an accurate and up to date Level One on their chart, to include level two’s when needed.
- Assure that the policy and procedure for grievances and complaints is adhered to and that grievance forms are completed and followed up with the appropriate documentation.
- Responsible for completing assessments, care plans and MDS sections pertaining to Social Services.
- Any suspected abuse must be reported to the Director of Nursing/Administrator immediately.
- Perform others tasks as assigned.

Educational Requirements and Experience:
- High school diploma required, Bachelor’s degree preferred.
- Related experience within the Health Care Industry.
- Proficiency in the use of pertinent computer software and systems.
Knowledge, Skills and Abilities:
- Skilled in motivating and directing the workforce.
- Ability to work cooperatively as a member of a team.
- Ability to communicate effectively with the residents and their family members, and at all levels of the facility.
- Ability to react decisively and quickly in emergency situations.
- Ability to maintain confidentiality.
