Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

Provide expert pharmacy services as a Home Infusion Pharmacist in a direct hire role located in Wetumpka, AL. This position involves delivering safe, patient-centered care in an alternate care and home infusion setting, Monday through Friday with typical hours from 8:30 AM to 5:00 PM or 9:00 AM to 5:30 PM, with occasional extended hours as needed.

Key Responsibilities: Ensure ethical and compliant pharmacy practices aligned with regulatory, accrediting, and professional standards. Assess patient eligibility for home infusion therapy and evaluate therapy appropriateness. Design and manage individualized care plans, setting therapy goals and monitoring effectiveness. Accurately dispense medications, supplies, and medical equipment for home infusion patients. Monitor lab results, perform pharmacokinetic dosing, and maintain detailed documentation of therapy progress. Educate patients and caregivers on medication administration and infusion techniques. Maintain USP compliance during sterile compounding and oversee clean room procedures. Supervise pharmacy support staff, interns, and students, delegating tasks as necessary. Participate in patient care meetings and provide drug information support. Share on-call duties to support patient needs outside regular hours.

Qualifications: Bachelor of Science in Pharmacy required; PharmD or Masters degree preferred. Active pharmacist license in Alabama with no negative history. Minimum of one year recent experience in home infusion pharmacy. Deep knowledge of IV admixture services and infusion therapy. Strong communication, problem-solving, and organizational skills.
