Schedule:
Full-time, Monday to Friday
Typical hours: 8:30am5:00pm or 9:00am5:30pm
(Some flexibility to stay later when needed)

Position Overview:
As a Home Infusion Pharmacist, youll provide safe, accurate, and compassionate pharmacy services to patients receiving infusion therapy in their homes. Youll oversee all aspects of the medication processfrom assessing therapy appropriateness to preparing and verifying sterile compoundswhile ensuring compliance with state, federal, and accreditation standards.

Key Responsibilities:

• Deliver high-quality, ethical pharmacy care in accordance with professional and regulatory guidelines.

• Evaluate patient suitability for home therapy and design individualized care plans with measurable goals.

• Prepare, verify, and dispense IV admixtures, medications, and related supplies.

• Monitor lab values, perform pharmacokinetic dosing, and document all clinical interventions.

• Educate patients and caregivers on safe infusion techniques and medication management.

• Ensure all sterile compounding practices meet USP <797> standards.

• Supervise and mentor pharmacy technicians, interns, and support staff.

• Collaborate in interdisciplinary care conferences and provide drug information as needed.

• Participate in on-call rotation and respond to patient needs after hours when required.

Qualifications:

• Active Alabama Pharmacist License (in good standing, no negative history)

• Bachelor of Science in Pharmacy required; PharmD or Masters preferred

• Minimum 1 year of recent home infusion experience

• Strong understanding of IV admixture services and infusion therapy

• Excellent communication, clinical judgment, and organizational skills
