Crowne Health Care is currently accepting applications for an Admissions Coordinator within our long-term care and rehabilitation facility in Thomasville, Alabama .

This position plays an important role in helping prospective residents and families navigate the admissions process while building strong relationships with referral sources, community partners, and facility staff. Qualifications & Requirements

• Working knowledge of admissions policies, procedures, and regulations within long-term care settings
• Minimum of one year of experience in admissions, nursing, healthcare administration, or a similar healthcare role preferred
• Long-term care experience strongly preferred
• Strong documentation, organizational, follow-up, and communication skills
• Ability to market the facility to prospective residents, families, referral sources, placement agencies, and the community
• Excellent interpersonal skills with the ability to build strong relationships with residents, families, referral partners, and staff
• Demonstrated professionalism, dependability, flexibility, and accountability
• Strong attention to detail and ability to manage multiple priorities
• Commitment to delivering excellent customer service and creating a positive admissions experience for residents and families

Essential Job Functions

• Coordinate the admissions process from initial referral or inquiry through admission
• Interview prospective residents and family members to obtain required admissions information and signatures
• Review and organize admission documentation to ensure required information is complete and accurate
• Provide professional and welcoming tours of the facility to prospective residents and families
• Promote Crowne Health Care’s services to prospective residents, families, referral sources, placement agencies, and community partners
• Communicate with hospitals, physicians, referral sources, residents, families, and facility staff throughout the admissions process
• Maintain accurate and up-to-date listings of residents, including level of care, room assignments, and admission/discharge status
• Coordinate resident admissions, transfers, and discharges while keeping appropriate parties informed
• Maintain required admission and discharge logs, daily and monthly census reports, and other admissions-related records
• Maintain knowledge of current Medicare, Medicaid, state, and federal regulations related to admissions and discharges
• Ensure admissions and discharge processes are completed in accordance with facility policies and applicable regulations
• Build and maintain strong working relationships with residents, families, referral sources, community partners, and interdisciplinary team members
• Assist with census development and facility marketing efforts
• Provide timely communication, documentation, and follow-up throughout the admissions process
• Ensure resident privacy, personal rights, and property rights are respected throughout the admission and transition process
