Overview
We are seeking a dedicated and compassionate Physical Therapist to join our team. In this role, you will be responsible for evaluating, planning, and implementing individualized treatment programs for patients to improve their physical function and quality of life. You will work in an outpatient home health setting for hospital patients , as well as with geriatric and clinic-based patients. Your expertise in patient care and rehabilitation will be essential in helping patients achieve their recovery goals.

Responsibilities

• Conduct comprehensive assessments of patients’ physical abilities and limitations.
• Develop personalized treatment plans based on individual patient needs and goals.
• Implement therapeutic exercises and modalities to enhance recovery.
• Educate patients and their families on treatment plans, exercises, and self-care strategies.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate documentation of patient evaluations, treatments, and progress notes.
• Participate in discharge planning to ensure continuity of care post-treatment.

Skills

• Strong knowledge of geriatrics, home health, clinic settings, and acute care practices.
• Proficient in patient care techniques and rehabilitation strategies.
• In-depth understanding of physiology and medical terminology relevant to physical therapy.
• Experience with discharge planning processes to facilitate smooth transitions for patients.
• Excellent communication skills to effectively educate patients and collaborate with healthcare teams.
• Ability to demonstrate empathy and compassion towards patients during their recovery journey.
• Strong organizational skills to manage multiple patient cases effectively.

Qualifications:
Graduate of an accredited Physical Therapy program
Current state license as an Physical Therapist
Minimum of 1 year of experience as an Physical Therapist
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Valid Driver’s License
Must provide a current CPR card
Must provide valid and current automobile liability insurance
Experience with the geriatric and pediatric population
Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, and lifting of patients and/or equipment up to 50 pounds.
