As a Social Services Assistant at Pine Haven Rehabilitation and Wellness Center, you will play a vital role in providing exceptional care and support to our patients and residents. Responsibilities:
• Assist the social worker in conducting assessments, developing care plans, and coordinating services to meet patients' and residents' social, emotional, and psychological needs
• Provide emotional support and counseling to patients, residents, and their families
• Facilitate communication between patients, residents, and healthcare providers to ensure seamless care transitions
• Coordinate community resources and services to enhance patients' and residents' quality of life
• Maintain accurate and timely records of patient and resident interactions, assessments, and care plans
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
