Job Summary
We are seeking a skilled and compassionate Admissions Nurse to join our team. The Admissions Nurse will play a key role in welcoming new residents, coordinating admissions, and ensuring a smooth transition into our facility. This position requires excellent clinical skills, attention to detail, and a commitment to resident-centered care.
Responsibilities
- Conduct initial assessments of new residents to determine care needs and develop individualized care plans.
- Coordinate with families, physicians, and other healthcare providers to gather necessary medical information and ensure seamless admissions.
- Educate residents and families about the facility’s services, policies, and care processes.
- Document resident information accurately in compliance with state and federal regulations.
- Collaborate with the interdisciplinary team to ensure continuity of care.
- Provide emotional support to residents and families during the transition process.
- Participate in quality improvement initiatives to enhance the admissions process.
Qualifications
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in [State].
- Minimum of [1-2] years of nursing experience, preferably in long-term care or geriatrics.
- Strong assessment and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of state and federal regulations governing nursing homes.
