We are currently accepting applications for an Admissions Coordinator & Social Services within our long-term care and rehabilitation facility specializing in geriatrics. This blended leadership role supports both the admissions process and the emotional and social needs of residents and families.

This position requires dedication, dependability, flexibility, accountability, and a positive attitude. Candidates must enjoy building relationships with residents, families, community partners, and interdisciplinary staff.

Qualifications & Requirements

  1. Working knowledge of admissions policies, procedures, and regulations within long-term care settings.
  2. Minimum of one year of experience in admissions, nursing, or a similar role in a long-term care or healthcare facility (preferred).
  3. Licensed Social Worker in Alabama (preferred).
  4. Education in Social Sciences or related field.
  5. Strong documentation, organizational, follow-up, and communication skills.
  6. Ability to market the facility to prospective residents, families, placement agencies, and the community.
  7. Excellent interpersonal skills with the ability to build strong relationships with residents, families, and staff.
  8. Demonstrated leadership ability and professionalism.
  9. Must be dependable, flexible, accountable, and demonstrate a positive attitude.
  10. Long-term care experience strongly preferred.
  11. Commitment to delivering excellent customer service and enhancing the highest practical quality of life for residents.

Essential Job Functions

  1. Interview residents and family members to obtain required admissions information, signatures, and social history details.
  2. Provide professional, welcoming tours of the facility while promoting Crowne Health Care’s mission and services.
  3. Act as a liaison and advocate for the resident’s emotional, social, and transitional needs throughout their stay.
  4. Assist in planning, organizing, implementing, and evaluating the Social Services program in accordance with federal, state, and local regulations.
  5. Maintain accurate and up-to-date listings of all residents, including level of care, room assignments, and admission/discharge status.
  6. Coordinate resident admissions, transfers, and discharges while keeping residents, families, and staff informed.
  7. Maintain required documentation such as admission/discharge logs, census reports, progress notes, and other departmental records.
  8. Ensure adherence to current Medicare and State regulations governing admissions, discharges, and resident rights.
  9. Build and maintain strong working relationships with residents, families, referral sources, and interdisciplinary team members.
  10. Advocate for and protect resident personal and property rights, ensuring all staff uphold these standards.
  11. Demonstrate exceptional customer service throughout the admissions process and in ongoing support of residents’ social and emotional needs.
  12. Provide timely documentation, communication, and follow-up related to resident care, psychosocial needs, and service coordination.
