Monday–Friday Day Shift | Minimal Direct Care | Clinical Analytics Focus

Are you an RN who enjoys solving problems, analyzing information, and identifying the details that others might miss?

Not every nurse wants to spend an entire career working bedside shifts. Some nurses are energized by reviewing clinical data, assessing resident needs, spotting trends, and using their nursing judgment to improve outcomes.

If that sounds like you, a career as an MDS Coordinator / Resident Assessment Coordinator (RAC) may be the right next step.

This is a Monday-through-Friday nursing position with a strong focus on resident assessment, documentation, clinical analysis, and care planning. Outside of completing resident assessments, the role is primarily administrative and does not require routine bedside care.

What You’ll Do

Under the direction of the Director of Nursing, the MDS Coordinator leads the interdisciplinary resident assessment process and helps ensure each resident receives an accurate, individualized plan of care.

You will use your clinical experience, analytical skills, and attention to detail to:

  1. Coordinate comprehensive assessments for assigned residents.
  2. Review clinical documentation and identify changes in resident conditions, needs, and functional abilities.
  3. Analyze resident information to ensure assessments accurately reflect the care being provided.
  4. Lead the development and completion of individualized care plans in accordance with company, state, and federal guidelines.
  5. Collaborate with nursing, therapy, dietary, social services, and other members of the interdisciplinary care team.
  6. Monitor care plans and recommend updates when a resident’s condition or needs change.
  7. Identify documentation gaps, inconsistencies, or trends that require follow-up.
  8. Serve as an internal resource and educator for team members involved in resident assessment and documentation.

This Role May Be a Great Fit If You:

  1. Enjoy working with data, documentation, and clinical information.
  2. Naturally look for patterns and ask questions when something does not add up.
  3. Have strong assessment skills and enjoy using critical thinking.
  4. Prefer a structured Monday-through-Friday schedule.
  5. Want to remain in nursing while moving into a role that is less physically demanding than traditional bedside care.
  6. Enjoy collaborating with different departments to improve resident outcomes.
  7. Take pride in accuracy, organization, and attention to detail.

Why Consider This Opportunity?

A Predictable Weekday Schedule

Enjoy a Monday-through-Friday day-shift schedule with greater flexibility than many hospital or bedside nursing roles.

Minimal Direct Care Responsibilities

Your primary direct-care responsibility is resident assessment. The majority of your work will focus on clinical review, documentation, coordination, and analysis.

Use Your Nursing Skills in a Different Way

This is an opportunity to apply your clinical background in a role that combines nursing judgment with data analysis, problem-solving, and regulatory knowledge.

Build Meaningful Resident Relationships

Unlike many acute-care environments, long-term care allows you to better understand each resident’s history, goals, progress, and changing needs over time.

Grow Your Career

This role can provide valuable experience in clinical reimbursement, compliance, quality improvement, and nursing leadership. You may also have the opportunity to pursue the Resident Assessment Coordinator Certification (RAC-CT) through AAPACN.

Qualifications

  1. Current Registered Nurse (RN) license in good standing.
  2. Strong clinical assessment and documentation skills.
  3. Excellent analytical, organizational, and deductive reasoning abilities.
  4. Working knowledge of nursing standards of care and medical practices.
  5. Comfortable using electronic medical records and documentation systems.
  6. Ability to communicate effectively and collaborate with an interdisciplinary team.
  7. Previous MDS, long-term care, or post-acute care experience is helpful but not required for the right candidate.

Benefits

We offer a competitive benefits package for you and your family, including:

  1. Competitive wages
  2. Blue Cross Blue Shield health insurance
  3. Dental insurance
  4. Vision insurance
  5. Life insurance
  6. 401(k) with company match
  7. Paid holidays
  8. Paid sick time
  9. Paid vacation

Join a team where your nursing knowledge, attention to detail, and analytical mindset can make a meaningful difference in the lives of our residents.
