Job Description:
Sunbelt Staffing is seeking a skilled Home Health RN for a travel contract in Florence, AL. This role requires experience with OASIS and Homecare Homebase (HCHB), along with strong clinical and organizational skills. The nurse will provide compassionate care in patients homes while ensuring thorough documentation and coordination with the care team.
Key Responsibilities:
Conduct in-home skilled nursing visits
Complete OASIS assessments accurately and in a timely manner
Document all care using Homecare Homebase (HCHB)
Educate patients and caregivers on care plans, medications, and health management
Communicate with physicians and interdisciplinary teams to ensure quality outcomes
Qualifications:
Current Alabama RN license
Minimum 2 years of RN experience, with at least 1 year in home health
Proficiency with OASIS and Homecare Homebase (HCHB) required
Reliable vehicle, valid drivers license, and auto insurance
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
