In this LPN long-term care role, your primary responsibilities include monitoring residents’ conditions, administering medications and treatments per physician orders, performing wound care, assisting with activities of daily living, and contributing to individualized care plans. You’ll conduct regular assessments, document changes, report urgent concerns, and coordinate with RNs, CNAs, therapists, social workers, and families to ensure cohesive, person-centered care.
