Responsibilities
Serves as a key team member within the central referral center, acting as a primary source of general health and clinical program information.
Provides accurate and efficient referral coordination for patients referred by or into the organization’s ambulatory services.
Makes outbound calls to patients and/or physician offices to obtain referral preferences and appointment details.
Handles general inbound calls from appointment request lines, patient navigation lines, and other referral sources from clinics and practices.
Manages referrals received via fax or electronic systems on behalf of clinics.
Delivers a high level of customer service as a primary entry point for patients contacting the health system.
Utilizes a variety of electronic healthcare systems to:
Review patient health information via EMR.
Schedule and pre-register patients.
Explain organizational and departmental policies.
Coordinate medical record collection and insurance verification.
Communicate with clinic staff via EMR and/or telephone.
Facilitate nurse triage access via EMR or live transfer.
Supports immediate access and resolution for patients seeking referral services.
Serves as referral and consultation support within the Referral Team and Care Access Department.
Manages the referral queue and ensures all referrals are linked to appointments.
Obtains necessary insurance plan information and tracks authorized referrals.
Schedules appointments consistent with patient needs and clinic guidelines.
Sends appointment confirmations and supporting information to patients as needed.
Maintains accurate appointment systems, including cancellations and reschedules.
Coordinates and schedules ancillary and consult appointments.
Completes authorization coordination for assigned referrals.
Supports online referral processes via web-based tools (e.g., referral portals, intake systems).
Reviews daily assigned schedules to determine required medical record actions.
Orders, coordinates, and distributes medical records per clinic-specific procedures.
Maintains communication with appropriate clinic personnel regarding medical records in line with departmental policies.
Answers general referral inquiries in a professional and courteous manner, following organizational customer service standards.
Provides general information to patients and callers, including program details, parking, and directions.
Maintains regular and prompt attendance and the ability to work a defined schedule and overtime as required.
Performs other related duties as assigned.
Qualifications
High school diploma or equivalent required.
Four (4) years of clerical experience in one or more of the following settings:
Access center
Customer service
Healthcare setting
Preferred experience with:
Databases and Electronic Medical Records (EMRs)
Referral tracking software
Precertification and referral management for insurance plans
