Assistant Case Manager - BLS
Position Summary:
The Assistant Case Manager provides support to individuals with Serious Mental Illnesses (SMI) in developing and maintaining basic living skills. The Assistant Case Manager plays a key role in helping consumers achieve greater independence by providing hands-on assistance with day-to-day tasks and ensuring access to essential services and appointments.
Key Responsibilities:
- Assist consumers in developing and maintaining basic life skills, including personal hygiene, meal preparation, and home organization.
- Provide support with household chores such as cleaning, laundry, and grocery shopping.
- Assist consumers with managing financial responsibilities such as budgeting, paying bills, and accessing benefits.
- Transport consumers to medical, mental health, and community appointments as needed.
- Promote the development of independent living skills in alignment with individual service plans.
- Maintain accurate documentation of services provided and consumer progress.
- Collaborate with the Case Manager and interdisciplinary team to support client goals and service coordination.
- Participate in regular supervision, team meetings, and case reviews.
- All other duties as assigned.
Qualifications:
- Education: High School Diploma or GED (required).
- Experience: Previous experience in a human services or mental health setting preferred.
- Strong communication and interpersonal skills.
- Ability to work with compassion, patience, and flexibility in a variety of community settings.
- Ability to maintain confidentiality and adhere to professional boundaries.
Transportation Requirements:
- Provide transportation for consumers as needed using agency or personal vehicles.
- Maintain a safe driving record and valid driver’s license with personal liability insurance.
- Follow all agency transportation policies, including seatbelt usage, safe cellphone practices, and vehicle documentation.
- Immediately report any vehicle issues, accidents, or violations to appropriate personnel.
- Must have reliable transportation, maintain a safe driving record and valid driver’s license with personal liability insurance.
Work Environment & Schedule:
- This is a non-exempt, full-time position.
- Standard hours are Monday through Friday, 8:00 AM – 4:30 PM CST or 8:30 AM - 5:00 PM EST, depending on the service area.
- This position requires in-person work and is not eligible for remote or hybrid work arrangements.
- Assistant Case Managers will provide services in community settings, including clients’ homes, public spaces, and other locations as needed.
- Must be able to travel within the service area on a regular basis and maintain a consistent in-person presence to meet consumer and program needs.
In-Service Training & Continued Education Requirements:
- Participation in all agency-required in-service trainings.
- Commitment to ongoing professional development to enhance skills and stay informed on best practices in behavioral health.
- Completion of onboarding and any continuing education requirements as directed by the agency or supervisor.
