Responsibilities
- Provide direct nursing care to residents according to the plan of care and facility protocols
- Monitor resident conditions, administer medications, and document vitals and responses to treatment
- Assist with activities of daily living and promote resident safety, comfort, and independence
- Administer treatments, manage wound care, and support infection control practices
- Collaborate with the charge nurse and interdisciplinary team; communicate changes promptly
- Maintain accurate and timely documentation in the patient record systems

Requirements
- Active LPN license or eligibility to practice as a practical nurse
- Ability to work a rotating night-shift schedule, including extended shifts as needed
- Prior clinical experience in a long-term care or skilled nursing setting preferred
- Proficiency in documentation and electronic health records; strong communication and teamwork skills
- Ability to work in a skilled nursing environment and adhere to all safety and regulatory requirements
