Hunter Creek Health & Rehabilitation is excited to welcome nurses to our growing team as we begin an exciting new chapter under the leadership of a new Administrator and Director of Nursing .

We're looking for compassionate, dependable LPNs who are passionate about providing exceptional care and want to work in a supportive environment where teamwork, respect, and resident-centered care come first!

Position Details

• Shift: 3:00 PM – 11:00 PM
• Pay: $25.00–$29.55 per hour (based on experience)

What You'll Do

• Provide compassionate, high-quality nursing care to residents in our skilled nursing and rehabilitation community.
• Assess, monitor, and document residents' conditions.
• Administer medications and treatments accurately and safely.
• Collaborate with physicians, therapists, and the interdisciplinary care team.
• Supervise and support Certified Nursing Assistants to ensure excellent resident care.
• Communicate effectively with residents, families, and fellow team members.
• Maintain accurate documentation while following all state and federal regulations.
• Help create a positive, safe, and welcoming environment for our residents.

What We're Looking For

• Current Licensed Practical Nurse (LPN) license
• Strong clinical and assessment skills
• Excellent communication and teamwork abilities
• Compassionate, resident-focused approach to care
