About the Role:

As a Restorative at Pine Hill Rehabilitation and Wellness Center, you will work closely with our interdisciplinary team to develop and implement individualized rehabilitation plans. Your expertise in restorative nursing will focus on helping patients maintain and improve their functional abilities, promoting a safe and effective transition to home or community-based care.

Responsibilities:

• Develop and implement individualized rehabilitation plans in collaboration with the interdisciplinary team
• Provide restorative nursing care to patients, focusing on ADLs, mobility, and patient safety
• Conduct resident assessments and creates care plans tailored to each patient's unique needs
• Collaborate with therapists and other healthcare professionals to ensure comprehensive care
• Monitor and report patient progress, communicating effectively with family members and healthcare teams
• Maintain accurate and detailed records of patient care and treatment plans
• Participate in quality improvement initiatives to ensure exceptional patient care and outcomes

Requirements:

While a high school diploma or equivalent is preferred, no specific level of education is required for this role. Previous experience in restorative nursing or a related field is beneficial, but not necessary. Please note that additional training may be provided for the right candidate.
