Brief Overview:

  2. Schedule: Full-time/ Monday - Friday
  3. Salary: $50,000 - $60,000 Annually

Position Overview: Social Worker

We are seeking a compassionate and dedicated Social Worker to support the emotional, social, and practical needs of our residents and their families. This role plays an essential part in promoting resident well-being, assisting with care planning, and ensuring a positive, supportive experience throughout each resident’s stay.

Key Responsibilities

  1. Plan, develop, organize, implement, evaluate, supervise and direct the social services programs and activities.
  2. Insure an ongoing quality assurance program, develop, and implement appropriate plans to correct deficiencies.
  3. Implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance quality of life.
  4. Consult with, coordinate social services activities with administration, medical and nursing services, consultants and other related departments, and implement changes as required.
  5. Ensure that necessary forms, reports and progress notes are properly completed and contain informative descriptions of the services provided and the residents’ responses to the services.
  6. Participate in community planning, development and implementation of social care plans, resident assessments and discharge planning to meet the needs of the facility, the residents and their families.
  7. Coordinate the social services programs with the residents, their families, staff members and community agencies through interviews, consultations and direct involvement in the programs.
  8. Provide information to residents and their families concerning available support agencies and financial assistance programs to meet the needs of the residents.
  9. Maintain incident and accident reports related to behavior management and elopement.
  10. Maintain effective relationships and open communication with residents, families, staff, community and outside healthcare resources.
  11. Participate in pre-admission planning and placement of residents in the facility.

Qualifications

  1. Bachelor’s degree in Social Work or related field required
  2. Experience in long-term care, rehabilitation, or healthcare setting preferred
  3. Knowledge of discharge planning processes and community resources
  4. Strong documentation, organizational, and communication skills
  5. Ability to manage sensitive situations with professionalism
  6. Understanding of regulatory requirements in skilled nursing environments
