Social Worker
Summary
The Behavioral Health Interdisciplinary Program (BHIP) in outpatient mental health provides team-based - veteran-centered - recovery-oriented - and evidence-based care. The incumbent in this position functions as a Mental Health Treatment Care Coordinator (MHTC) for Veterans on BHIP teams and is responsible for coordination and continuity of care for Veterans receiving services throughout Mental Health (MH).

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
- United States Citizenship
- Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
- Education
- Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
- Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited
- Licensure
- Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level

- One year of experience equivalent to the GS-11 grade level as listed below
- Experience must demonstrate possession of advanced practice skills and judgment - demonstrating progressively more professional competency
- Knowledge of community resources - how to make appropriate referrals to community and other governmental agencies for services - and ability to coordinate services
- Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic - educational - and other backgrounds
- Knowledge of medical and mental health diagnoses - disabilities and treatment procedures (i.e
- acute - chronic and traumatic illnesses/injuries - common medications and their effects/side effects - and medical terminology) to formulate a treatment plan
- Skill in independently implementing different treatment modalities in working with individuals - families - and groups who are experiencing a variety of psychiatric - medical - and social problems to achieve treatment goals
- Ability to provide consultation services to new social workers - social work graduate students - and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment

- Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level - and must be able to provide supervision for licensure

- In addition to the experience above - the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation - negotiation - and monitoring
- Knowledge and ability to write policies - procedures - and/or practice guidelines for the program
- Ability to supervise multidisciplinary staff assigned to the program
- Skill in organizing work - setting priorities - meeting multiple deadlines - and evaluating assigned program area(s)

Duties
- The Behavioral Health Interdisciplinary Program (BHIP) in outpatient mental health provides team-based - veteran-centered - recovery-oriented - and evidence-based care
- The incumbent in this position functions as a Mental Health Treatment Care Coordinator (MHTC) for Veterans on BHIP teams and is responsible for coordination and continuity of care for Veterans receiving services throughout Mental Health (MH).The incumbent is a Social Worker assigned to a BHIP team at the Central Alabama Veterans Healthcare System (CAVHCS)
- The BHIP care coordinator is responsible for providing care coordination services including but not limited to biopsychosocial assessment - education - provision of self-management resources - and other clinical tasks as needed for Veterans receiving services in BHIP
- Care coordination (including care management and/or case management) includes determining and monitoring care coordination needs over time - providing care coordination - and facilitating follow-up and outreach as needed to promote proactive - personalized care and care continuity
- Total Rewards of a Allied Health Professional Duties include but not limited to: Coordinates care including serving as an advocate on behalf of the Veterans on BHIP teams to ensure delivery of comprehensive - seamless MH service
- Actively participates in program planning and development - advising the BHIP team of shifting trends with care coordination needs and caseloads and identifying the need for making recommendations on program procedures and/or policy changes based upon work experiences
- Takes responsibility for improving professional skills - participating in planning and executing specific staff development projects - attending seminars and institutes - and participating in training programs of other disciplines as requested
- Provide care coordination (including care management or case management) to Veterans and families - as needed - throughout the continuum of care
- This includes communication within and across care lines to facilitate continuity of care for Veterans
- Provide consultation/education to Veterans and families on VA resources (including self-management tools) - community resources - Advance Directives and VA benefits
- Work Schedule: Monday - Friday 8:00am - 4:30pm

- When setting pay - a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade)

- Child Care Subsidy: After 60 days of employment - full time employees with a total family income below $144 -000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
