Join our team at Altoona Health and Rehab as a Restorative Nurse! Located at 6532 Walnut Grove Rd., Altoona, Altoona Health and Rehab is dedicated to providing high-quality care and support to our residents. We are currently seeking a skilled and compassionate Restorative Nurse to lead our restorative nursing program.

As a Restorative Nurse, you will play a vital role in promoting resident independence, mobility, and overall well-being. You will work closely with our interdisciplinary team to develop and implement individualized restorative care plans for residents and oversee the delivery of restorative services.

Key Responsibilities of a Restorative Nurse:

Assessing resident needs and developing individualized restorative care plans to promote independence and functional ability.

Implementing restorative nursing interventions, including range of motion exercises, ambulation programs, and activities of daily living training, to maintain and enhance resident mobility and function.

Providing direct care and supervision to residents participating in restorative programs, ensuring safety and adherence to care plans.

Monitoring resident progress and adjusting restorative interventions as needed to achieve desired outcomes.

Documenting resident assessments, care plans, interventions, and outcomes accurately and timely in accordance with regulatory standards and facility policies.

Providing education and training to nursing staff and certified nursing assistants on restorative care principles and techniques.

Collaborating with therapists, physicians, and other healthcare professionals to coordinate care and optimize resident outcomes.
