If you have a desire and passion to serve the rehabilitation and healthcare needs in both the long-term and short-term care, to work in a fast-paced geriatric rehabilitation environment, this is the opportunity for you!Minimum Responsibilities include but are not limited to:To act as a liaison and representative of the residents' social interestsTo plan, organize, develop and direct the overall operation of the Social Services department in accordance with current federal, state and local standards governing the facility, and as may be directed by the Administrator, to ensure that the medically related emotional and social needs of the residents are met and maintained on an individual basisAssist in developing residents a preliminary and comprehensive assessment and written care plan that identifies the emotional, psychological needs and strengths and/or needs of the residents, assess on an ongoing basisMust establish and maintain relationships with resident and families regarding their rights and responsibilities, encourage residents to participate in the development and review of care plansMinimum Qualifications:Must be at least 18-years of ageBachelor's degree is Social Work or related fieldMust have two years of previous experience as a Social WorkerPrevious SNF or hospital experience preferredMust be able to communicate effectively, both verbally and in writingMust maintain the confidentiality of the residents, comfort and positive attitude in communicating with the elderlyJob Type: Full-TimeBenefits: Competitive Pay, Comprehensive Benefits ProgramInterested in joining our team?Monroe Manor Health & Rehabilitation Center236 West Claiborne StreetMonroeville, AL 36460 - Phone: (51) 575-2648Monroe Manor Health & Rehabilitation Center is an Equal Opportunity Employer
