Job Summary

Priority Care is hiring a Care Coordinator to work directly inside our partner clinics. This role focuses on enrolling patients into our care management programs (CCM, PCM, and RPM) and helping them stay engaged in their care between visits.

You’ll be working closely with clinic staff and speaking with patients daily, explaining the program and getting them set up. This is a hands-on, patient-facing role where building relationships and being comfortable starting conversations is key.

Responsibilities

• Identify patients who may qualify for care management programs
• Talk with patients about the program and enroll them
• Get patients set up in the Priority Care system/app
• Work directly with providers and clinic staff throughout the day
• Be the main point of contact for Priority Care inside the clinic
• Keep track of enrollments and basic documentation
• Follow up with patients as needed
• Help the program run smoothly within the clinic

Qualifications

• Experience in a healthcare setting (medical assistant, care coordinator, front office, or similar role preferred)
• Strong communication and interpersonal skills
• Comfortable speaking with patients about healthcare services
• Ability to work independently within a clinic environment
• Organized and detail-oriented

Preferred Skills

• Experience with patient enrollment or care coordination programs
• Familiarity with EMR systems
• Experience in patient-facing roles (healthcare, customer service, or similar)
