Position Summary

The Clinical Liaison serves as the primary connection between Aspire and hospitals across the Hoover and Birmingham area , driving admissions through strong referral relationships and seamless care transitions.

What You’ll Do

1. Build and maintain relationships with hospital case managers, discharge planners, and physicians
2. Evaluate and coordinate incoming referrals for admission
3. Serve as the clinical and operational liaison between hospitals and the facility
4. Guide patients and families through the transition process
5. Conduct on-site hospital visits and community outreach
6. Track referral activity and adjust strategy to meet census goals
7. Partner with facility leadership to execute growth strategies

What We’re Looking For

1. LPN preferred (clinical background highly valued)
2. Experience working in hospital case management, admissions, or post-acute care
3. Strong relationships within the Birmingham/Hoover healthcare market preferred
4. Proven ability to build trust and influence referral decisions
5. High-energy, self-motivated, and results-driven
6. Excellent communication and follow-up skills
