Job Summary

The Case Manager (RN, LPN, or Social Worker) supports patients across the Hoover and greater Birmingham, Alabama area , coordinating care transitions, discharge planning, and access to post-acute services—similar to a hospital case management role, but with a more predictable schedule and manageable caseload.

Responsibilities

1. Coordinate care transitions and discharge planning
2. Collaborate with nursing, therapy, physicians, and families
3. Arrange home health, DME, rehab, and placement services
4. Connect patients with Birmingham-area community resources
5. Manage documentation and ensure timely, accurate records

Qualifications

1. Active Alabama license (one of the following required):
2. Registered Nurse (RN)
3. Licensed Practical Nurse (LPN)
4. LMSW or LCSW (Social Worker)
5. Experience in case management, discharge planning, or care coordination preferred
6. Strong communication, organization, and problem-solving skills
