Job Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to residents while collaborating with Registered Nurses, physicians, and interdisciplinary team members to ensure quality resident outcomes and compassionate care delivery.
This role supports both long-term care residents and rehabilitation patients in a team-focused clinical setting.
✅ Responsibilities
• Administer medications and treatments as prescribed
• Monitor and document resident conditions and changes in status
• Communicate effectively with supervising nurses and physicians
• Support quality resident care and infection control practices
• Maintain accurate and timely documentation
• Provide compassionate care and support to residents and families
• Work collaboratively with the interdisciplinary care team
Qualifications
• Current Licensed Practical Nurse (LPN) license in good standing
• Strong clinical and communication skills
• Ability to work effectively within a team environment
• Compassionate, patient-centered approach to care
• BLS/CPR certification required
