**Description** **Registered Nurse - Home Health** **Per Diem/Relief | Day Shift | Florence, Oregon** **Hourly Range: $49.30 - $77.46** **This position requires some weekend shifts.** Bring your nursing expertise and compassion to a place where your care truly matters. **PeaceHealth** is seeking a **Registered Nurse - Home Health** for a **Per Diem/Relief** position on the stunning **Oregon Coast in Florence, Oregon** . You'll assess, plan, coordinate, and evaluate patient care while empowering patients and families through education and support. Every visit you make helps promote healing, independence, and peace of mind. **What You'll Do** + Conduct comprehensive patient assessments and determine eligibility for home health services. + Develop and manage individualized care plans in collaboration with physicians and the interdisciplinary team. + Deliver high-quality, compassionate nursing care and provide instruction to patients and caregivers. + Coordinate care with Home Health Aides and other clinical partners to ensure seamless delivery of services. + Document all care accurately and promptly in compliance with agency and regulatory standards. + Support families and caregivers with education and bereavement resources when needed. **What You Bring** + Graduation from an accredited school of nursing required; **BSN preferred** . + **Two years** of acute care or home health/hospice nursing experience required. + Current **RN license** in Oregon, **BLS certification** , **driver's license** , and proof of auto insurance required. + A passion for delivering patient-centered care and the ability to work independently in a home setting. **Why PeaceHealth?** + Hourly compensation: **$49.30 - $77.46** , dependent on experience and collective bargaining agreement. + Work in a supportive, mission-driven environment guided by compassion, collaboration, and respect. + Experience the beauty of coastal Oregon while serving your community. **Join us in bringing peace, healing, and exceptional care to every patient-one home at a time.** PeaceHealth is committed to the overall wellbeing of our caregivers.
