Responsibilities
- Assess residents’ PT needs and develop individualized treatment plans aligned with functional goals.
- Deliver PT interventions including therapeutic exercises, gait training, balance activities, range of motion, and pain management.
- Monitor progress, document outcomes, and adjust plans based on patient response and interdisciplinary input.
- Educate patients and families on home exercise programs, safety, and disease/condition management.
- Collaborate with the interdisciplinary team to support discharge planning, goal attainment, and coordination of care.
- Maintain accurate documentation in electronic health records and ensure compliance with regulatory and facility policies.

Requirements
- Active Physical Therapist license in Alabama or eligible for licensure
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Ability to work in a skilled nursing facility setting and participate in a travel assignment
- Willingness to commit to a 13-week assignment
- Strong communication skills and ability to collaborate with a multidisciplinary team
- Proficiency with electronic health records and standard PT documentation
