QA Receptionist

Reports to Chief Quality Officer

Background: Cahaba Medical Care Foundation is a community health center organization providing medical, pharmacy, dental, and behavioral health services to diverse underserved communities in Jefferson, Bibb, Perry, and Chilton counties. We are a Level 3 Patient-Centered Medical Home and Joint Commission-accredited organization, committed to increasing integration and coordination of behavioral health and primary care. Responsibilities & Duties

• Proactively contact patients following Emergency Room visits to schedule timely follow-up appointments

• Coordinate appointment scheduling in accordance with provider availability and clinical priorities

• Obtain medical records from hospitals and emergency departments, including discharge summaries, test results, and treatment notes

• Accurately upload and document external medical records into the organization’s Electronic Medical Record (EMR) system

• Communicate professionally with hospital staff, patients, and internal clinical teams to ensure continuity of care

• Track and document outreach efforts, appointment status, and received records

• Maintain patient confidentiality and comply with HIPAA and organizational policies

• Identify and escalate urgent follow-up needs to the appropriate clinical staff when necessary

• Conduct regular outbound calls to efficiently coordinate appointments for patients, focusing on essential services such as Medicare Wellness visits and pap smears, among other important health screenings.

Qualifications:

• Have proficiency in general computer operations and data entry.

• Be able to respect and maintain the confidentiality of all information, following HIPAA protocols.

• Be able to carry out responsibilities with little or no supervision and work as a team player.

• Have a professional attitude and appearance.
