Your Role:

1. Conduct thorough physical exams and manage acute and chronic illnesses in a geriatric population
2. Create and manage individualized care plans that consider each resident’s full medical, emotional, and physical needs
3. Prescribe medications, order diagnostic tests, and perform procedures within your NP scope
4. Collaborate with physicians, nurses, social workers, and other team members for holistic, coordinated care
5. Educate residents and families on health issues, treatments, and care options
6. Serve as an advocate for resident well-being
7. Maintain accurate documentation in the EHR

Qualifications:

1. Certified Nurse Practitioner (NP), preferably with geriatric experience
2. At least 2 years of clinical experience in long-term care, nursing home, or similar setting
3. Strong clinical and diagnostic skills
4. Excellent interpersonal and communication skills
5. Empathy-driven, patient-first mindset
6. Proficiency in electronic health record (EHR) systems
