Shift: MondayFriday days with weekend rotation and on-call as needed

Job Description:
Sunbelt Staffing is hiring a Home Health RN for a 13-week travel assignment in Sylacauga, AL. The role requires strong clinical judgment, the ability to manage an independent caseload, and proficiency with OASIS and Homecare Homebase (HCHB). Nurses in this role will provide compassionate care and support to patients and families in their homes.

Key Responsibilities:

• Conduct skilled nursing visits and assessments

• Complete and submit OASIS documentation in compliance with agency timelines

• Maintain accurate records in Homecare Homebase (HCHB)

• Educate patients and families on disease management and home care plans

• Work collaboratively with the interdisciplinary care team

Qualifications:

• Current Alabama RN license

• 2+ years of RN experience, including at least 1 year in home health

• Experience with OASIS and Homecare Homebase (HCHB) required

• Reliable transportation, valid drivers license, and auto insurance

• Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
